Here is an ADHD
test to give you an idea if you may have AD/HD for your
information only. This test can give you some idea if you should get a
full evaluation done with a professional. If you live in the San
Jose Ca. area call 408-243-5541 for more information. This ADHD test is
for adults, children, and teens. Print this out, take
it, keep the results. Then take it again after you have tried your
therapy or ADHD comprehensive treatment program. It will give you a baseline
from which to measure your progress.

Test Additional Considerations
-
Some hyperactive-impulsive and inattentive symptoms that caused
impairment were present before age 7 years.
-
Some impairment from the symptoms is present in two or more
settings (e.g., at school or work and at home.)
-
There must be clear evidence of clinically significant impairment
in social, academic or occupational functioning.
-
The symptoms do not occur exclusively during the course of a
Pervasive Developmental Disorder, Schizophrenic or other Psychotic
Disorder and are not better accounted for by another mental disorder
(e.g., Mood Disorder, Anxiety Disorder, Dissociate Disorder or a
Personality Disorder.)
General Adult ADHD
Test Symptom Checklist
In conjunction with other diagnostic techniques the following general
adult ADHD checklist helps further define ADHD symptoms. No ADHD adult
has all of the symptoms, but if you notice a strong presence of more
than 20 of these symptoms, there is a strong likelihood of ADHD.
Please read this list of behaviors and rate yourself (or the person
who has asked you to rate him or her) on each behavior listed. Print the
text below and then, using the following scale, and place the
appropriate number next to the item. The scoring procedure is at the end
of the test.
0 = Never 1 = Rarely
2 = Occasionally 3 = Frequently
4 = Very Frequently
IMPORTANT: This is not a tool for self-diagnosis.
Its purpose is simply to help you determine whether ADHD may be a factor
in the behavior of the person you are assessing using this checklist. An
actual diagnosis can be made only by an experienced professional.

All
our products naturally stimulate the body and brain by adding the
nutritional building blocks necessary for rebalancing biological systems
which maintain bio-chemical consistency in the brain. Chemical
imbalances in the brain are at the root of AD/HD symptoms. Our
philosophy is basic nutrition is not available, even in the best of
diets, to meet the needs of individuals susceptible
to AD/HD symptoms.
The amino acids, vitamins and minerals required to
help produce neurotransmitters
in sufficient quantities associated with the brain's biochemical
deficiencies in ADD/ADHD are available in one multivitamin-amino acid formula called beCALM’d.
